OBJECT_NAME(parent_object_id) AS TableName,
type_desc AS ConstraintType
FROM sys.objects
WHERE type_desc LIKE '%CONSTRAINT'
For specific table:
add the following to very end:
AND OBJECT_NAME(parent_object_id)= 'theNameOfYourTable'
